Should I fill up the Scholarship Supplement Form?

I have recently been admitted to UIUC without scholarship.

There is a link on http://admissions.illinois.edu/faq/admitted.html for a scholarship supplement form.

Should I fill it up if I want to apply for merit based aid? The problem is, on the like above, it says

And in the form instructions, it says:

So, if I want some merit based aid, should I fill up the form or not? Note that I haven’t filed any FAFSA.

Call financial aid and ask. Highly unlikely that you would be denied any type of aid simply by filling out a form. If anything is unclear, however…call. That’s what they staff for.

I did, I don’t see how it will hurt, and it takes like five minutes.
