<p>I fell ill with pneumonia twice during the third year of college. Mostly during exams. It effected my grades very badily. 6th semester grade was the lowest grade I got out of all the 8 semesters.</p>

<p>yes, you should as an explanatory note for your poor performance that semester. However don’t dwell on it. Instead, simply note that a serious illness resulted in anomalously low academic performance. Your LOR writers might also make a note of that in their letters.</p>
<p>That doesn’t matter - if one of your other letter writers is your advisor or research supervisor, they might know you well enough that they can discuss your illness and note that your performance that semester does not reflect your abilities.</p>
