<p>OK, I give up. I see that the printed pdf copy has a place for an electronic signature, but there wasn’t a place to enter that on the form that is submitted on line. Any clues? Thanks!</p>

<p>Do you just check those little boxes? Why does it look so different on the print preview? We can’t be the only ones wondering about this. I’m also thinking that if we missed something it would have come back and told us. We got a confirmation at the end. Thanks!</p>
