Staying consistent and steadily increasing?

Hi. I want to try and break 1400 on Math and CR combined. My last two scores for CR were 690 and 700 from practice tests out of the blue book. My last two scores for Math were 650 and 670. So there are two things I want mainly want advice on:

After getting to 700 on CR, how can I stay consistent in the 690-710 range? I haven’t studied a lot of vocabulary. In fact, I’ve really only studied Direct Hits Volume 2 and some random lists from the internet. Would studying more vocabulary help? I typically miss around 2-4 SC questions, and the same goes for passage-based reading. I usually miss 2-4 (sometimes more) in those as well.

How can I get those last 30 points in Math? I usually get tripped up in the last few questions of a section, sometimes completely missing the last 2-3 questions of a section. I usually do best in the 18, then the 20, and I usually miss the most in the 16.

I’m going into my senior year. Since I’ll be going to a community college and then transferring, I figure I can take the SAT in November and there won’t really be an issue in applying. So what is the most effective way to reach those two goals I have mentioned above in the time that I have? Thank you in advance for anyone willing to help me out.
