<p>Yesterday evening, I sent a message to all University students to notify you of the inadvertent use of Social Security numbers in a recent Student Health Insurance Plan brochure mailing. The letter will contain general information and instructions for enrolling in a complimentary, one-year credit monitoring service.</p>

<p>You are a student who has been affected by this incident. I wanted to follow up with you directly and provide precautionary measures that you may take to protect your personal information. If you do not wish to wait for your letter to arrive via U.S. mail, you may contact the toll-free call-in center beginning on Monday, July 22, to receive your unique activation code for the complimentary credit monitoring service. If you live in North Carolina or Massachusetts, your unique activation code will be available on Tuesday, July 23. You will need your unique code to sign up for the credit monitoring service. We encourage you to take advantage of this service. Students and parents may also discuss any questions or concerns with the center staff. To contact the call-in center, please call 1-877-216-8137, Monday through Friday, 9:00 a.m. to 7:00 p.m. Eastern Standard Time. When prompted, please enter the following 10-digit reference code: xxxxxxxxxx</p>
