What does it mean if a student (not an athlete, but had perfect scores) was invited to interview on campus during the summer, did so, and then is contacted by an alum and urged to also have an alumni interview? Is it routine for Yale to request both? Does this mean the on campus interview was not positive, and Yale wants a second opinion? Or is it routine for academic admits?

I think it just depends on the number of alumni interviewers in your home area–if there are enough/you got your application in early while there were still a lot of spots for alumni interviews then you’ll get one. I also had an on campus interview the summer before my senior year and an alumni interview after I applied.
I don’t think offering this extra interview says anything about whether the application is good of bad, i would take the second interview since it was offered.

Goolsci is right. It just means that there are enough alumni interviewers in your available to interview the student. Except in very rare situations, the admissions office doesn’t not make special requests of alumni interviewers to make sure someone is interviewed. That alumnus the student talked to was almost certainly urging the student to take the alumni interview because s/he is an enthusiastic person, not because of any special knowledge of the student.
