The diagnoses? The symptoms?
Both, I think. Colleges need to know the disability and any difficulties that a student might face because of them. Do the symptoms affect or impede the student’s ability to learn? Because that is the kind of information that universities want. Also, confer with the student’s counselor on how to fill specific sections of the supplement.
