Supplemental Recs

I want to send in supplemental recommendations from a professor I do research with. Do I just give him the teacher evaluation form? Does it have to be labeled as a supplemental recommendation? Can it be done online or does it have to be mailed?

Online or mail doesn’t matter as long as it gets there.
I’m not too sure whether he qualifies as a “teacher” evaluating you as a student though.
You should label it as a “separate” recommendation, I think I’ve seen that in the supplemental form somewhere.

Again, not too sure. When in doubt, always call them.

Yeah, ditto davidbonko. Do not give him a teacher recommendation. Just have him mail his letter to Yale and label the envelope or something as supplemental recommendation.
