Teacher Recommendation???

<p>For the teacher recommendation, can it be sent on school letterhead? Or does the teacher have to fill out the secondary school report? My AP Bio teacher does not know my rank and all of that (my counselor is already completing the report) so does he have to fill that form out? Or can he just send in a letter of recommendation with my USC ID on it?</p>

<p>For any recommender who fills out the form, a letter of recommendation is supposed to be attached to it. (That instruction is in the fine print on the form.) School letterhead is the preferred stationery.</p>

<p>It is fine if the teacher just puts “N/A” or a line through any questions that they do not have sufficient information to complete. Letters can also be sent without the form (assuming you have at least one recommender filling out the form), just be sure your name is properly spelled (you would be surprised how often recommenders spell names incorrectly!) and your USC ID# or birthdate is included.</p>

<p>As I understand it, you have to give one of those counselor/teacher report forms to each person you plan to write a recommendation for you. So yes, your AP Bio teacher would also have to fill out one of those forms.</p>

<p>I also asked my AP Bio teacher to fill one out for me, she put the form and a recommendation letter in a sealed envelope for me to mail. I plan to fax the reports from my counselor after Thanksgiving break.</p>
