<p>hey guys, im going into my senior year and my tetanus vaccine is gonna expire, i have an appointment to get the shot but on my compliance status it says i need to fill out a medical history & physical examination report. i did this freshman year, do i need to do it all over again or just give record of my tetanus vaccine?</p>
