I eventually got eye surgery to fix the problems (better than the brain surgery they thought I needed!) and my senior year grades are back to my normal standards. I put a note about it in the application, and while I hate using an “excuse”, my vision problems really did impact my grades and I wanted to make sure there was no confusion with slacking vs. not being able to see.
