<p>What does it say/what do you have to do to fill it out? Is your signature required?
Im just curious, because I havent recieved it yet, and I’m wondering how long it takes to fill out.</p>

<p>It is very short and simple, although i do believe it requires your signature. The big kicker is the $400 they want with it ;o</p>

<p>The thing is probably smaller than an index card, and you just pretty much sign and send, no big deal.</p>
