What application would I need for this situation?

<p>I left “school 1” in 1/2009 w/ 58 credits attained. I want to go back to school, but let’s say I want to go to “school 2”.</p>

<p>What type of application would I have to fill out for “school 2”?</p>

<p>That would usually be a transfer application.</p>
