What is a Perkin's Promisory Note?

I noticed on my school’s financial aid website that to accept a loan, I need a Perkin’s promisory note. Unfortunately, I have no idea what that is, and no further information was provided. :[

I tried looking it up, and it appears to be a federal form used for financial aid by several colleges. Where can I get one, and how should I fill it out? What is it used for, exactly?

Thanks so much if anyone can help. :]

Call the financial aid office. The PPM is a document you sign that has the terms of the loan - signing it is your promise to repay it. Although my school has electronic promissory notes for Stafford loans, we require a paper Perkins promissory note (although it is available for download on the Forms page of our Financial Aid section on our school website). Your question is a simple one for your school to answer … so call.
