when to send the fee-waivers and what if i had a 4th language?

I was wondering when to send the fee-waiver and could i upload it or should i send it by mail??
And, i have 4 languages so where to put the fourth one?
And, about the school forms, my school isn’t used to the process ,so everything is by paper unless i could ipload the documents ,so what to do?</p>
