<p>I’ve logged onto the Document Management System where it gives you the option to “download forms” and electronically upload them, but when I click on “download forms,” a blank webpage comes up. I have no idea why it’s not letting me access the forms. Does anyone else have a clue? Or if someone has already downloaded a PDF of Form 5 that you could possibly send to me? I really need to get this in. Thanks.</p>
<p>Nevermind. I found a PDF of it on their website. Can a mod delete this thread for me? Or maybe I can do that myself and am too stupid to know how…lol.</p>

<p>^ Oh my counselor and teachers have already sent in recommendations. I was talking about Form 5, the test score report (unofficial). I ended up uploading it electronically, with some painstaking effort but they’ve sent me a receipt saying they received it so hopefully they’ve got everything. Thanks anyway!</p>
