<p>I really can’t find the Supplemental Document Cover Sheet. Can anyone tell me how to get there once I’m on my MyMIT?</p>
<p>Yeah sure … Just click on Secondary School Report and Transcript Tab … you will get on the page “Freshman Application Forms” … scroll down to where it says sending additional docs … click on the link " MIT Supplemental Document Cover Sheet" … hope it works !!</p>
<p>I need the document cover sheet as well. It is not available on MyMIT. I have already had a person write an additional letter, but I cannot send it in. MITChris or anyone else who can help please reply.</p>
<p>In addition if someone has saved the document as a PDF please let me know. I will be bump the FAQ thread as well.</p>
<p>anyone having the pdf file may not be helpful mate cuz there will be Name and MIT_ID already printed on the PDF … isn’t My method working up ?</p>
<p>I don’t know if everyone else has the exact same problem, but when I log in for myMIT, only Part 1, Part 2, and the Midyear Report are highlighted as links. I don’t see any “Freshman Application Forms” links…</p>
<p>I found it by Googling the exact name.</p>
<p>I tried that, but if you look closely that happens to be the cover sheet for transfer applicants, not freshman applicants.</p>
