which fax form is for me to send application form only

<p>I believe USC has already received all my supporting materials.
So, do I only need to mail/fax the application forms to USC?
which fax form should I use? If I want to mail the application forms to them, is 12/1 postmark not too late? Do I need to have a form(fax form) to attach my application forms in the mail?</p>

<p>Boy! I thought I only need to submit on line!</p>

<p>If you are able to submit online, there is no need to FAX /mail the application. FAXing/mailing the application is a last-ditch-backup-plan-in-case-of-emergency-contingency-plan if you cannot get through online.</p>
