Why are my scores so low?

I have taken the SAT thrice and the ACT twice, yet I still end up having “sub-par” results. My IQ is very high, American Mensa, 95 average GPA U/W, 97ish U/W after this year so what is up with my scores? I do have ADD and test anxiety but I don’t take medicine for is which could be apart of the problem. If anyone has any answers of recommendations then please tell me. I would love to get my scores up. I am looking into test optional schools in the event that I can not get my test scores up/

What kind of scores are we talking here? American Mensa is not “very” selective - 98%ile and above.

Here are the familiar standards (approximate)

ACT: 29
PSAT: 180
SAT: 1300
