Dear Dean Dr. name and Members of University
My cordial salutation, my name is ---------- and I have been a student here at University since 2014 I am writing this letter to appeal my academic dismissal from University BSN Program. I’d first like to thank you for this opportunity to be heard in regards to my dismissal from University, I’m grateful to be given a chance to appeal and plead my case. In the Spring term 2016, I receive a “D” in NSG3009 Principle of Assessment and a D in NSG3022 Pharmacotherapeutics I, admit I was having very difficult time after my provider inform me that my Xray images show I had many masses, my husband was very sick at that time, was a very hard time for me and my family and my academic performance was affected. Doing my reinstatement I have always held myself to a certain standard of performance and this past semester does not reflect that. I hold myself accountable for this, Even though I tried I was not able to maintain my grades to the standard that both the University, and I, would have liked, my poor grades were not on NSG3046 Caring for adults II. It was not due to low ability or knowledge or because school is not my priority, Instead my grades have been suffering because I have been very sick, I believe that my academic performance and my illness, severe headache, sleeping issues, swelling stomach, fever, chest pain, discomfort in upper abdomen and anxiety, were directly related to my issues of being sick, I started feeling very sick the second week of the semester I tried my best to manage. On Tuesday March 27, 2018 I felt terrible, I had chest pains throughout the day, I experienced migraines, and also upper stomach pain, however I took some medication still managed to finished my OB final. As I was studying for my Med Surg with my private tutor in the afternoon I started feeling Chest pain, back pain, dizziness, abnormal heartbeat, again at 3 A.M. In the morning of Wednesday March 28, 2018 and so again I took some medication due to the fact that I was having chest pain, severe headache, shortness of breath dizziness, I fell and my family took me to the emergency room, my family call professor D, also they emailed her to explain why I could not attend school that day, I already have a doctor note for the school boards convenience to have on file. The doctor diagnosis me with chest wall pain, costochondritis, epigastric pain, GERD (gastroesophageal reflux disease), elevated blood pressure reading without diagnosis of hypertension, I didn’t know that I have GERD, or Hypertension, or any heart problem I was eating some of the foods that was against some of my diagnostic, now I know I receive the proper treatment as well as medication and I believe i’m on a steady ground to start again. I have since been feeling better day by day and I am grateful for the opportunity that God has given to me, I have strive to do better, I have recovered I workout to help destress, I also make sure to get at least 8 hours of sleep. My clinical OB instructor in Dr. M know I was not feeling well, I was sent back home from clinical because I was having fever for many days, I already meet with professor D and review what I was doing wrong and how I can improve, I’m very responsible with school always on time never missed a day, always gave my assignment on time and dedicated to my classes, I go to tutoring, I recorded class, I use all my resources. Overall, I have turned a new page on working to better my health mentally and physically as a human being I have now have a much better understanding of my healthy, workload. I am fully committed to raising my grades, my school is very important for me. I Understand that I have slipped below the standards of this prestigious University but I am implementing changes to improves my performance. I respectfully request that you review my case and reconsider my request for readmission into the nursing program thank you in advance for taking the time to overview this letter.

You should have someone thoroughly review this letter for grammar and usage. It sounds like you had a rough time but the punctuation, sentence structure and grammar are not doing you any favors.
