<p>I have emailed an advisor in my major to try to schedule a meeting, but before all this, I was wondering what the Declaration of Major form looks like. I couldn’t find it online and I know I can get it at Monroe Hall, but I won’t make it to school until Saturday (1st semester 
yay!, although technically 5th, I’m a transfer student) and I was wondering if any of you guys had a copy or remembered what it looks like and what kind of information it asks for. Is it necessary for me to meet with an advisor to fill it out? Does he need to sign it or something, or can I just do it on my own?</p>

<p>You technically complete the bottom half with the advisor, but I just filled it out beforehand and had my advisor sign off on it.</p>
