<p>All new full-time students must complete an on-line CSF by October 1, 2009. Those students who do not complete an online CSF by October 1, 2009 will automatically be enrolled in the NU Plan and will be charged for a full year’s premium. There will be NO appeal process for students who do not have an online CSF on file by the deadline date.
HOW TO SUBMIT AN ON-LINE COVERAGE SELECTION FORM (CSF)</p>

<p>Log into your CAESAR Account using your Net Id and Password. On the left side of the computer screen you will click on “For Students” or “For Applicants.” Next, click on “Enrollment or Health Coverage,” then on “Health Coverage Selection Plan.” Make your selection of either enrolling into the Aetna Plan or waiving out of the Plan, and submit. You will receive a confirmation e-mail stating that your completed form has been received. If you do not receive a confirmation e-mail, please contact the Student Insurance Office at (847) 491-2113.</p>
