this is embarassing to ask, but I might have lost the original counselor form

<p>So now near graduation I’m trying to find the form that I thought I specially safekept – you know, the one they mailed out in April, that asks your secondary school counselor all sorts of information and asks him or her to send it after you graduate. Are there any replacements I could print out online?</p>

<p>Doesn’t your school automatically send a final transcript out to your chosen college?</p>

<p>Yeah, but I think there were some questions on that form that I can’t recall off the top of my head.</p>
